The Medical Records department collects, distributes, stores, and retrieves medical information from the medical record maintained on each patient at the Health and Wellness Center. All information is strictly confidential and is released only through written consent by the patient.
We will not disclose Protected Health Information about our patients without prior written consent from the patient on a per request basis. For patients under 18, parents may request most medical information (call for details).
