As an extension of our customer service, the Student Health and Wellness Center files medical insurance claims for the University endorsed student insurance policy underwritten by United Healthcare Student Resources.

The Student Health and Wellness Center is an in-network provider for the University endorsed student insurance policy underwritten by United Healthcare Student Resources. We are currently working towards becoming an in-network provider for many nationally known insurance carriers.

Insurance Brochure and Enrollment Forms

Information regarding the University endorsed student insurance policy, underwritten by UnitedHealthcare, may be obtained at the Student Health and Wellness Center or online at http://www.uhcsr.com.

Enrollment forms for the University endorsed student insurance policy are mailed with payment to:
